We recently announced being a recipient of the Trillium Community Health Plan (Trillium)’s Community Benefit Initiative grant.
The $936K Community Benefit Initiative grant recognizes Fora Health’s commitment to enhancing community health and well-being. The grant funds will help the organization support unhoused patients in recovery to access housing with rental payments assistance, security deposits, home furnishings, utility expenses and food needs. The grant also helps create an opportunity to provide Fora Health’s unhoused patients with complete whole-person care, affording them the opportunities to become fully independent in a new housed environment and accrue resources necessary to keep them from falling back into houselessness – a serious issue facing the Portland metropolitan area.

Aimed at driving health equity and access in Oregon, the Community Benefit Initiative grant supports local organizations that demonstrate a dedication to addressing critical health issues and improving the overall health of the community. These strategic investments are rooted in Trillium’s commitment to improve health outcomes and address fundamental social factors impacting health, such as stable housing, food security, social supports and educational opportunities.

The Community Benefit Initiative grant was awarded to 54 community organizations and nonprofits in the Tri-County and Southwest regions of Oregon. Grant recipients were selected based on each project’s ability to implement evidence-based community-level interventions that improve health and healthcare quality, prioritize collaborative partnerships to amplify impact, and advance health equity for the community at large.
