The breach was quickly contained and while no explicit evidence of stolen data was found we are taking this breach very seriously. There was no financial information included in the breach. Any patients who could have had exposed personal information viewed will receive a separate notification and we urge anyone with questions to reach out to Fora Health at email@example.com.

How To Log Into Tele-Counseling
Current patients & families, please make sure you read through the tele-counseling instructions document provided by your counselor for detailed information on how to access your scheduled sessions. Fora Health uses the Zoom video conference platform for all virtual counseling sessions. We chose Zoom to ensure confidential and secure sessions. Current patients will need to download the Zoom app before the start of your session. Zoom requires an email address to use the app and counselors will use that email address to send you links for upcoming counseling sessions.

In order to have a tele-counseling session, current patients must be using a device that has a working microphone. Many laptops come pre-installed with webcams and microphones and most smartphones and tablets have cameras that can act as webcams.
