Walk-In Withdrawal Management (Detox) Admission Process

Individuals seeking withdrawal management (commonly called detox) should arrive by 7:30 am Monday through Saturday at our facility at 10230 SE Cherry Blossom Dr., Portland, OR 97216.  Saturdays we are only able to accept OHP patients.

Withdrawal Management patients will be admitted after a brief triage, with all those presenting on time given equal consideration for a bed. There is no guarantee of admittance; it depends on bed availability and individual medical evaluation.

We accept the Oregon Health Plan (OHP) and most other insurance providers.* Saturdays we are only able to accept OHP patients.

*If you are privately insured OR living outside the Clackamas, Multnomah or Washington County areas and are seeking withdrawal management services, please call us at (503) 535-1151 Monday through Friday, 7:30 a.m. – 4:30 p.m. before arriving at the facility for enrollment.

Drop-In Outpatient Medications to Treat Addiction (MAT) Hours 

Fora offers same day, drop-in and telephone appointments for individuals seeking to manage their addiction with medications:

Do you live in Washington, Clatsop, Columbia, Tillamook or Yamhill county?  If YES, call or drop-in to the Hillsboro office to talk with a MAT coordinator.

• Hillsboro Location Telephone:  (503) 713-9155
• In-person:  Monday through Friday:  9:30 a.m. – 11 a.m. for a same day consultation with the program coordinator.
• MAT office hours are 9:30 a.m. – 5:00 p.m. but availability is limited.

Do you live in Multnomah or Clackamas county?  If YES, currently the pathway to receiving MAT services is to first engage with Withdrawal Management (Detox) services at the Cherry Blossom location.

• In-person:  For Withdrawal Management Sevices (Detox) show up at Fora Health at 7:30am Monday through Saturday.  On Saturdays we can only accept individuals who have Oregon Health Plan Insurance (OHP).
• Patients may enroll in MAT services before discharging from Withdraw Management (Detox).  Patients also are able to enroll in MAT services through Outpatient services. 

New Patients: We do accept new patients who have recently left Fora Withdraw Management program within the last 3 weeks.

Established patients:  If you have missed an appointment or need to be seen before your next scheduled appointment: call  in during drop-in hours.

Same Day Walk-In Outpatient Assessments 

To get established in our Outpatient care, individuals interested in enrolling in care can arrive onsite during walk-in hours to start the enrollment process which will include an assessment and assignment to an Addiction Counselor at the SE Portland or Hillsboro location.

Enrollment Walk-In Hours*:

Tuesdays:  2 p.m. to 4 p.m.
Thursdays: 10:30 a.m. to 2 p.m                                                                                  Fridays: 11 a.m. to 12 p.m.                                                                                  (Friday hours start August 9, 2024-Cherry Blossom location only)

*Expect a minimum 2.5 hour appointment.  There may be additional wait time.

Individuals can call (503) 535-1151 during regular business hours to speak to a patient support associate for additional information.

We accept a variety of payment options including Oregon Health Plan (OHP) and work with many insurance providers.
