Clinical Program Director

As Clinical Programs Director at Fora Health, Meredith is responsible for overseeing both the Residential and Outpatient programs. She is dedicated to effective program development and fostering teamwork across various disciplines. Meredith prioritizes the continued delivery of trauma-informed, evidence-based services to our patients by Fora’s exceptional clinical staff.

Meredith’s journey in the Substance Use Disorder (SUD) field began in 2013 when she embarked on her career as a Client Relations Associate at Fora Health, formerly known as De Paul Treatment Centers. Since then, she has ascended to various leadership roles across different levels of care within the SUD treatment continuum. Meredith’s academic credentials include a Bachelor’s degree in Community Development from Portland State University’s School of Urban Planning and a Masters of Human Services from Warner Pacific College. She holds CADC II and QMHP certifications from MHACBO.
