Charlotte Van Cleve

Charlotte Van Cleve serves as the Director of Psychiatric Services at Fora Health, where she provides clinical leadership and oversight of psychiatric care across residential, ASAM 3.7, and outpatient programs. She plays a key role in ensuring high-quality, integrated psychiatric services that support individuals in recovery from substance use and co-occurring mental health disorders.

Charlotte began her career with Fora Health, formerly De Paul Treatment Centers, in 2014 as a Client Support Specialist at the front desk, gaining firsthand experience in patient-centered care. She progressed through several roles, including Medical Case Coordinator, Detox Technician, Psychiatric Mental Health Nurse Practitioner (PMHNP), and Medical Manager of Psychiatric Services, before assuming her current leadership position. She began practicing as a PMHNP in 2019.

She holds a Bachelor of Science and a Master of Science in Nursing and is board-certified as a Psychiatric Mental Health Nurse Practitioner (PMHNP-BC). Charlotte also holds the Certified Addiction Registered Nurse – Advanced Practice (CARN-AP) credential, reflecting her expertise in addiction medicine and integrated behavioral health care.
