Donor of the Year: Diane & Richard Van Grunsven. The Van Grunsvens have been long-time supporters of treatment in our community and De Paul/Fora Health. They have inspired generosity at events, and supported initiatives that translated directly to saving lives! In particular, we are so grateful for their significant and generous support to help Fora Health build our facility!

This year, we have been able to start an amazing life-changing program at Fora Health through Trillium Community Health Plans’ Community Benefits Initiative. These strategic investments are rooted in Trillium’s commitment to improve health outcomes and address fundamental social factors impacting health, such as stable housing, food security, social support, and educational opportunities.
