Fora Health will be closed on Wednesday, December 25th in observance of the Christmas holiday. Services will continued to be provided for the Residential and Withdrawal Management (Detox) patients in our care, however we will not admit to any programs, provide outpatient services, and our lobbies are closed.

It is Fora’s 50th Anniversary and it is time to celebrate!
Since our inception as De Paul Treatment Centers in 1974 we have been committed to providing treatment, care and advocacy for all who are affected by substance use disorder, helping to transform over 55,000 lives. This milestone marks a momentous journey filled with stories of hope from patients, families, the remarkable dedication of employees and the continual support of our community partners. This anniversary not only signifies the years of providing care for the community but also, the immense potential that lies ahead.
Throughout this year, we will be highlighting stories of how individuals, families, employees, and the community have been positively impacted by Fora Health/De Paul Treatment Centers. This will all lead up to our 50th Anniversary Hope and Freedom Gala.
2024 Hope & Freedom Gala

Fora Health will host a Gala Dinner to showcase stories from donors, staff, and patients who’ve been apart of this transformational treatment. The Samuel C. Wheeler Freedom Award and Spirit of Advocacy Award will be presented to community members to recognize their support and work toward ending the stigma of addiction.
